The PPAC is a telephonic patient care unit whose staff provides vital information to patients before their surgical, cardiac, radiology and GI procedures. The staff in this department provide comprehensive patient evaluations as well as educate patients regarding anesthesia services and provide critical instructions. Pre-procedure assessment includes the identification of any medical problems that need to be addressed prior to surgery, and communicates test results to appropriate providers. The goal of their activities is to reduce day of procedure/surgery cancellations. Documentation of all information obtained is in the EHR. Schedule Details: Hours of operation for Pre-Procedure Assessment Center are 0645-1730. Typical 8 hour shift is from 0900-1730 but may be flexed to 0645-1515, 10 hour shifts would be from 0645-1715. NO WEEKENDS, NO HOLIDAYS What You Will Do: The PPAC Registered Nurse calls a variety of patients to complete a phone interview which includes updating EPIC medical record, providing instructions preoperative and pre procedure, as well as coordinates the test results between patients' providers. Part Time with Benefits FTE: 0.60 What You Will Do: Promotes positive patient outcomes for patients and families of ProHealth Care utilizing pharmaceutical care. The pharmacist incorporates and demonstrates clinical skills and customer service strategies in order to achieve the organization’s goals. Responsibilities: • Enter or verify physician medication related orders. • Verify/ approve the work of Pharmacy Technicians • Follows guidelines of the P&T committee for therapeutic interchange, and formulary management. • Monitors drug therapy for appropriateness and promote/ evaluate rational drug therapy that improves pharmaceutical care. Performs pharmacokinetic consultation. • Improves patient care by shortening length of stay, preventing ADR and controlling pharmaceutical costs • Will work with physicians, specialists and other clinicians to implement appropriate guidelines and protocols for drug use based on evidence based medicine, national guidelines, and literature based protocols. • May act as a preceptor to pharmacy students or pharmacy residents. • Provides mentoring for pharmacy students on rotation consistent with college requirements. • Provides drug information to nurses, physicians, and other pharmacists to ensure current information on drug therapy. • Participates in Performance Improvement (PI), Drug Use Evaluations (DUE), Adverse Drug Reactions (ADR) and clinical programs of the pharmacy to meet departmental and hospital goals. • Recommends changes to drug therapy based on patient assessment. • Provides and documents clinical interventions, therapy recommendations and other clinical activities according to department policy and procedure in order to maintain accurate and complete patient records • Participates in the Drug Reconciliation activities of the Pharmacy Department. • Participates in hospital committees as assigned. • Performs all other related duties as assigned by the supervisor. Our Lab services operate out of state of the art facilities and include clinical testing in the areas of Histology, Cytology, Hematology, Coagulation, Clinical Chemistry, Urinalysis, Immunoassay, Microbiology, Transfusion service, EIA, and Molecular Biology. ProHealth Care offers a vibrant outreach program, providing laboratory testing to numerous specialty hospitals, physician offices, and nursing home facilities. Our Medical Director, a member of the Great Lakes Pathology group, oversees the Laboratory and works closely with leaders and employees to monitor quality, identify, access, and implement new technology. The Laboratory has certification and accreditation through the College of American Pathologists (CAP) Joint Commission (JCAHO), American Association of Blood Banks (AABB) and Clinical Laboratory Improvement Act (CLIA). ​Hours: This is a 1.0 FTE primarily day shift with one clinic close shift per week ending at 1815. This position works every third weekend and has a rotating holiday schedule.
