Bluegrass Care Navigators (previously Hospice of the Bluegrass) is a nationally respected organization which provides care for the seriously ill. Having cared for thousands of families in our Kentucky communities since 1978 through hospice services, the company serves approximately 850 patients daily in 32 counties located in central, northern and eastern Kentucky.

Our services include:

  • Bluegrass Extra Care – a private pay service offering personalized home care
  • Bluegrass Home Primary Care - comprehensive, ongoing expert care for the "home-limited"
  • Bluegrass Adult Day Health Care - engaging day program for adults with physical or intellectual limitations
  • Bluegrass Transitional Care – help with hospital to home
  • Bluegrass Palliative Care – pain and symptom relief
  • Bluegrass Hospice Care – expert care for the final months of life
  • Bluegrass Grief Care – support through anticipatory grief and bereavement

Bluegrass Care Navigators operates a 12-bed hospice in-patient unit in Lexington at Saint Joseph Hospital, a 10-bed in-patient unit in Lexington at the University of Kentucky Medical Center, a 6-bed stand-alone unit in Eastern Kentucky and offers services located within partner health systems and facilities in KY. The agency also provides hospice services to patients at Baptist Health Lexington.

The American Hospital Association presented the 2017 Circle of Life Award™ “for innovation in palliative care and for improving the care of patients near the end of life”. Bluegrass Care Navigators is the only repeat honoree in the history of the award.

Provides the medical and psychosocial evaluation of patient and family; skilled and meticulous symptom management; extensive involvement in follow-up care, and discharge planning and continuity of care efforts. Receives all new referrals to palliative care, assesses the referral for appropriateness, and processes according to program policies.
