Having cared for thousands of families in our Kentucky communities since 1978 through hospice services, the company serves approximately 850 patients daily in 32 counties located in central, northern and eastern Kentucky.

Bluegrass Care Navigators operates a 12-bed hospice in-patient unit in Lexington at Saint Joseph Hospital, a 10-bed in-patient unit in Lexington at the University of Kentucky Medical Center, a 6-bed stand-alone unit in Eastern Kentucky and offers services located within partner health systems and facilities in KY. The agency also provides hospice services to patients at Baptist Health Lexington.

Bereavement Counselor – Full-Time Bluegrass Care Navigators – Southeastern, KY service area (Pike, Floyd and Magoffin) Bluegrass Care Navigators is seeking a licensed therapist with counseling experience in grief, loss and trauma. In this position the Bereavement Counselors will conduct assessments and provide supportive counseling to assist clients during their grieving process through individual, group and family sessions. The Bereavement Counselor will be expected to provided community education on grief, collaborate in program development for events held throughout the year. Summary of Job Responsibilities: An employee in this position provides counseling and supportive services to help meet patient/family social and emotional needs in relation to the impact of a terminal illness and loss and grief. The bereavement counselor will work under the direction of the Counseling Supervisor, Executive Director and/or the Regional Counseling Supervisor. RESPONSIBILITIES AND DUTIES In conjunction with members of the interdisciplinary team, assess patient/family social and emotional factors in order to estimate their capacity and potential to cope with the terminal illness and potential death. Provides bereavement support and information to individuals coping with grief through mailings, one-on-one counseling, group counseling, and/or referral to community resources. Provide direct counseling and support services on an individual basis to patient/family in all categories of hospice care, including crisis intervention. Establish treatment goals and revise counseling interventions as indicated. Assist patient/family and staff in utilizing community resources. Assist team members in understanding the significant social and emotional factors related to care of the terminally ill patient and their families. Attends team meeting to stay aware of patient/family needs as terminal illness progresses. Participates in the orientation of new staff and in-service training and educational programs for professional staff. Participates in agency and community programs as requested to promote professional growth and understanding of hospice care (i.e., bereavement camps, Holiday Hope programs, memorial services, etc.). Provides consultation to other staff regarding special problems or treatments in relation to bereavement care. SUMMARY OF JOB RESPONSIBILITIES The Clinical Excellence Specialist has accountability for positively impacting the quality and completeness of care provided as evidenced by clinical documentation. The Specialist implements a comprehensive education program for his/her assigned areas of responsibility. The Specialist leads, in collaboration with the clinical manager/supervisor, Professional Development Director, Clinical Educators, existing onboarding materials, and ongoing competency assessments. The Specialist conducts audits, utilizes audit reports and provides education in the areas of opportunity and risk. The Specialist reports the results of audits and education to clinical, operations, quality and compliance management for use in the QAPI program. The Specialist demonstrates professional responsibility and fosters teamwork and a supportive, learning culture. The Specialist seeks to improve processes and works collaboratively with clinical managers/supervisors to educate, train, and implement process improvements based on findings. Other services may include contributions to the organization through ad-hoc or permanently established councils, committees and workgroups. SUMMARY OF JOB RESPONSIBILITIES An employee in this position performs a variety of tasks designed to assist coordination of the electronic medical record of each hospice patient from admission through discharge. This includes running reports, data analysis, document scanning and data entry. RESPONSIBILITIES AND DUTIES Sets up initial active medical record for Fayette County and creates a skeleton and cost file of the Fayette County, Cynthiana, Frankfort, and Jessamine County offices. Requests past medical records for all admissions and then verifies receipt of records with appropriate tracking mechanisms. Sends all change orders to appropriate physicians for signature and maintains a log to verify date sent and received. Sends physician agreements and standing orders to appropriate physicians for signature and maintains log for verification of receipt. Mails the care plan oversights/monthly summaries to appropriate physicians. Provides assistance in coverage of Medical Record Clerk II and the Information Systems Trainer in his/her absence. Ensures appropriate information is received by The Organization’s Care Center on any patient that is transferred from a home or nursing home team to the inpatient unit in the absence of the Medical Records Clerk II. Our team provides unsurpassed private duty nursing, palliative care, hospice care and more. Position Qualifications: One year general nursing experience is preferred. New nursing graduates are encouraged to also apply and will be considered along with other life experiences and interest in working in hospice field. Active KY state RN license CPR certification, negative TB skin test, pre-employment physician’s health clearance Summary of Job Responsibilities: The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of Bluegrass Care Navigators. Position offers the opportunity to enhance quality of life by providing one-on-one care to patients in their homes or other care environments at end-of-life. Empower caregivers and loved ones through individualized education, support, and skilled nursing. Provide specialized/individualized/customized care in unique (person centered) environments Demonstrate confidence in nursing skills including a comfort in operating independently in patient homes or other care environments. Utilize critical thinking skills in dynamic and unique environments to assist in meeting patient care needs at end-of-life. Work ratio of one-to-one providing education, safety, and symptom management to individuals enrolled in hospice care. Provide primary care within a supportive team network. This is a PRN position. TITLE: Pharmacist REPORTS TO: Pharmacist in-charge STATUS: Exempt QUALIFICATIONS Bachelor of Science Degree in Pharmacy from an accredited college of pharmacy, PharmD preferred. A minimum of two years experience in a healthcare setting. Supervisory experience preferred. Experience working with an interdisciplinary team. A current non-restricted license to practice pharmacy in the state. Knowledge and basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary healthcare team. Current Kentucky driver’s license with unlimited access to a reliable vehicle. Ability to exercise judgment and discretion in confidential matters. No positive drug or alcohol test including, pre-employment, random, post-accident or reasonable suspicion. Must be able to drive for extended periods of time with no ill effects. Must be flexible and have good written and oral communication skills. Possess and maintains good physical stamina and mental health. Has presented a pre-employment physician’s health clearance including a negative TB skin test and/or CXR and other tests as required by organization policy. SUMMARY OF JOB RESPONSIBILITIES An employee in this position will perform dispensing Pharmacist duties.
