LPN/MEDICAL HOME COORDINATOR: The Primary Care Neighborhood LPN is responsible for providing delegated nursing care to assigned populations. Utilizes a collaborative interdisciplinary approach to anticipate, integrate, and coordinate the delegated patient care. The goal of the intervention is to meet the needs of patients in order to optimize clinical and resource outcomes, and to contribute to the effectiveness of the care plan and its outcomes. In collaboration with the physician, the nurse identifies at risk patient populations who require additional services. Provides bio-psychosocial information to help identify patient needs and to contribute in the development of the plan of care. This role serves as a patient/family advocate and acts as a communication link between the physician, patient, family, outpatient and inpatient health care team, as well as community resources, to reduce redundancy and fragmentation of care.
